This form serves not only as a way to ensure that you are fairly compensated for any out-of-pocket costs but also helps maintain transparency, accountability, and proper budgeting.  We appreciate your understanding and cooperation as we roll out this new reimbursement policy. 

Simply download the form and fill it out no later than a week after post-event. Email it over with your related receipts and send to [email protected] (cc’ing [email protected]) for approval.
